登革热(dengue)是登革病毒经蚊媒传播引起的急性虫媒传染病。登革病毒感染后可导致隐性感染、登革热、登革出血热,登革出血热我国少见。典型的登革热临床表现为起病急骤,高热,头痛,肌肉、骨关节剧烈酸痛、部分患者出现皮疹、出血倾向、淋巴结肿大、白细胞计数减少、血小板减少等。本病主要在热带和亚热带地区流行,我国广东、香港、澳门等地是登革热流行区。由于本病系由伊蚊传播,故流行有一定的季节性,一般在每年的5~11月份,高峰在7~9月份。在新流行区,人群普遍易感,但发病以成人为主,在地方性流行区,发病以儿童为主。

登革热病因
登革病毒经伊蚊叮咬进入人体后,在毛细血管内皮细胞和单核-巨噬细胞系统内复制,然后进入血液循环形成次病毒血症。定位于单核-巨噬细胞系统和淋巴组织中的登革病毒继续进行复制,再次释入血流形成第二次病毒血症,并引起临床症状与体征。机体产生的抗登革病毒抗体与登革病毒形成免疫复合物,激活补体系统,导致血管的通透性增加,亦可导致血管水肿和破裂。登革病毒的复制可YZ中白细胞和血小板的再生,导致白细胞血小板减少和出血倾向。
登革热和登革出血热的不同临床表现与病毒的变异有关。通过塔希堤、斐济等太平洋岛屿的流行病学观察,发现不少初次感染的登革热病人也出现登革出血热临床经过,病人的血清反应也属初次感染类型,且儿童占多数。有人认为登革病毒感染的临床病情轻重与病毒的毒力有关。登革病毒通过变异产生毒力更强的病毒株可能是登革出血热发生的重要原因。

The replication of dengue virus can inhibit the regeneration of leukocytes and plaets in the bone marrow, resulting in leukocyte thrombocytopenia and bleeding tendency.
The different clinical manifestations of dengue fever and dengue hemorrhagic fever are related to the variation of the virus. Through the epidemiological observation of the Pacific Islands, such as Tashi dike and Fiji, many dengue fever patients were found to have a clinical course of dengue haemorrhagic fever, and the sera reaction of the patients was also the first type of infection, and the majority of children were infected. Some people believe that the clinical severity of dengue virus infection is related to the virulence of the virus. Dengue virus can produce virulent virus strains through mutation, which may be an important cause of dengue hemorrhagic fever.
