造血干细胞移植
取患者自身、异体或脐血转输给患者, 移植物中的多能干细胞可在体内定
居、增殖、分化, 使患者恢复造血功能和形成免疫力。该疗法可用于ZL免疫缺陷病、
再生障碍性贫血以及白血病等。
(一) 自体移植
白血病患者在化疗或放疗后输入ZL前采集的自体细胞, 中的多能干细胞
可迅速增殖、分化成各系血细胞, 重建机体的造血系统和免疫系统。在处理患者自身骨
髓时, 应尽可能杀死其中残留的白血病细胞, 或者纯化出 CD34+造血干细胞以用于回
输。重症联合免疫缺陷病表现为 T 细胞、B 细胞的发育障碍, 病因之一是腺苷脱氨酶
(ADA) 基因缺失。将患者 CD34+造血干细胞进行体外腺苷脱氨酶基因转移后回输, 可
明显改善症状。
(二) 异体移植
异体移植要求供者与受者的 HLA 型别相同, 才能成功, 否则会发生受者抗移
植物的排斥反应, 或因供者中的 T 细胞对受者同种异型抗原发生应答, 而产生后果
严重的移植物抗宿主病。
(三) 脐血干细胞移植
脐血干细胞的免疫原性较弱, 来源方便, 可在一定程度上代替同种异体移植。
