免疫YZ剂
免疫YZ剂能YZ机体的免疫功能, 常用于防止移植排斥反应的发生和自身免疫性疾病的ZL。
(一) 化学合成药物
1 . 糖皮质激素
糖皮质激素具有明显的KY和免疫YZ作用, 对单核/ 巨噬细胞、T 细胞和 B 细胞都有较强的YZ作用, 常用于ZL炎症、超敏反应性疾病和移植中排斥反应。
2 . 环磷酰胺
环磷酰胺属烷化剂型抗肿瘤药物, 其主要作用是YZ DNA 复制和蛋白质合成, 阻止细胞分裂。T细胞和 B 细胞活化后进入增殖、分化阶段, 对烷化剂敏感, 故烷化剂可YZ体液免疫和细胞免疫。环磷酰胺主要用于ZL自身免疫性疾病、移植排斥反应和肿瘤。
3 . 硫唑嘌呤
硫唑嘌呤属嘌呤类抗代谢药物, 主要通过YZ DNA、蛋白质的合成, 阻止细胞分裂, 对细胞免疫、体液免疫均有YZ作用, 常用于FZ移植排斥反应。
(二) 微生物制剂
1 . 环孢素
环孢素 (cyclosporin A, CsA, 环孢菌素 A) 是真菌代谢产物, 主要通过阻断 T 细胞内 IL - 2 基因的转录, YZ IL - 2 依赖的 T 细胞活化。环孢素用于ZL移植排斥反应有明显效果, 也用于ZL自身免疫性疾病。
2 . 他克莫司
他克莫司 (FK - 506) 属大环内酯类抗生素, 为真菌代谢产物。其作用机制与环孢素相近, 但作用比环孢素强 10～100 倍, 对于抗移植排斥反应有良效。3 . 西罗莫司
西罗莫司 (雷帕霉素) 属抗生素类免疫YZ剂, 可能通过阻断 IL - 2 启动的 T 细胞增殖而选择性YZ T 细胞, 用于抗移植排斥反应。 |
