患者腹痛的程度和体征不相符，常常腹痛剧烈，但体征轻微。发病初期腹部喜按，但随着胆道炎症的发生而出现拒按。2.恶心、呕吐呕吐物多为胃内容物，可含胆汁，也有可能吐出蛔虫。据报道呕吐出的蛔虫可多达数十条甚至上百条之多，小儿由于咽喉反射敏感性较差，可从鼻腔中爬出蛔虫。3.寒战、发热胆道蛔虫患者的体温多在正常范围之内，当合并感染时，患者可出现畏寒、发热，但体温的上升与腹痛的程度不成比例。蛔虫钻入胆道引起胆管炎，除非虫体退出胆道，炎症缓解，体温多不会自退。4.黄疸单纯的胆道蛔虫因虫体表面光滑，不易形成完全性胆道梗阻。但蛔虫钻入胆道后，若蛔虫的数量多、蛔虫死在胆道内，或反复发作的胆道蛔虫引起胆管炎时，可引起胆道梗阻，在梗阻后24～48小时可出现黄疸。
